If this is your first order:

1. Decide which forms you need and the quantity that is best for you (we recommend a nine month supply). Fill out the order form. Order form is provided in PDF (Adobe Acrobat) format.

2. Determine which window envelope you need for your cheques or statements. Window envelopes save you time and money, and will guarantee, you get the right document to the right employee.

3. Email or fax your order to: Viceroy International Forms, fax:(416) 538-0747, Viceroy E-mail:cs@viceroyforms.com. Shipping and handling charges will be added to your invoice. Viceroy will complete and return promptly the order form for your review and approval. We accept payments by VISA or Mastercard.
