How to choose earnings statement

Direct Deposit reporting forms

To help you choose the correct Direct Deposit reporting forms (otherwise called Earnings Statement) for your EasyPay software, answer the following question:

Do you need a detailed Year-to-date Earnings Statement or
This pay period non-detailed Earnings Statement?

This one page chart shows how to select the form that is right for you. It depends on printer that you will be using, way of distribution and your reporting requirements.

For detailed Earnings Statement choose form:

  • CS091(B) or (C) or CS096 (for impact printer) or
  • LS095(B) (Direct Deposit reporting laser stock sheet form with horizontal perforation for laser printer, prints two detailed statements per page) or LS093(C) (Direct Deposit reporting laser stock sheet form with horizontal perforation for laser printer, prints 3 non-Detailed Statements, or 1 Customized or Detailed Statement, per page)

For This Pay Period non-detailed Earnings Statement choose form:

  • CS091(A) or (B) or CS096 for impact printer or
  • LS093(A) for laser printer.

If you are using other software for your Direct Deposit reporting, we can look after your needs.

Note: Our form’s symbol is coded as follows:
C – continuos form (for impact/ dot matrix printer);
L – laser form (for laser printer)
S – stock (blank) form;
P – imprinted form (for instance with your company’s name and logo)
(A) – prints on 1/3 of a page (size 8 1/2″ w x 3 1/2″ d)
(B) – prints on 2/3 of a page (size 8 1/2″ w x 7″ d)
(C) – prints on full letter size page (size 8 1/2″ w x 11″ d)
For instance LS095(C) is a laser blank form 095 for detailed or non-detailed statement which printout length is 11″.
